What, really, can one say? Except that it would've been even more interesting if instead of clowns they were a pair of tragedy/comedy masks.

Hey I like the idea of clowning around with boobs, It is one of my favorite pastimes.
This is not as bad as you think since you have no clue about the cultural context.
First, German. Boobs are everywhere and uncensored. The Germans practically also are all nudists. So a boob here is not a boob there.
Second, this is an album of Karneval (Mardi Gras) songs. In the German tradition, people during Karneval time dress up as clowns, hence the use of clown masks. So, there is a cultural connection with regard to the clown masks being used as pasties.
The fact that you just posted this and expected everyone to share your bewilderment as to the use of the clown masks shows just how much of a rank amatuer you are with regard to cultural contexts, PhD notwithstanding.
For the record, cis gay male, so you now have a reason to dismiss my comment.

I like the album "swing that gospel axe" that shows a clean-cut up with people wannabe group standing in a field holding axes. I term this genre: ScaryCute. Supposed to be cute, but very scary. Or the scary things made to be scary are really too cute.
